Risk Factors for Surgical Site Infections Following Neurosurgical Spinal Fusion Operations: A Case Control Study

OBJECTIVE
To determine risk factors for the development of surgical site infections (SSIs) in neurosurgery patients undergoing spinal fusion.
DESIGNRetrospective case-control study.
SETTINGLarge, academic, quaternary care center.
PATIENTSThe study population included all neurosurgery patients who underwent spinal fusion between August 1, 2009, and August 31, 2013. Cases were defined as patients in the study cohort who developed an SSI. Controls were patients in the study cohort who did not develop an SSI.
METHODSTo achieve 80% power with an ability to detect an odds ratio (OR) of 2, we performed an unmatched case-control study with equal numbers of cases and controls.
RESULTSDuring the study period, 5,473 spinal fusion procedures were performed by neurosurgeons in our hospital. With 161 SSIs recorded during the study period, the incidence of SSIs associated with these procedures was 2.94%. While anterior surgical approach was found to be a protective factor (OR, 0.20; 95% confidence interval [CI], 0.08–0.52), duration of procedure (OR, 1.58; 95% CI, 1.29–1.93), American Society of Anesthesiologists score of 3 or 4 (OR, 1.79; 95% CI, 1.00–3.18), and hospitalization within the prior 30 days (OR, 5.8; 95% CI, 1.37–24.57) were found in multivariate analysis to be independent predictors of SSI following spinal fusion. Prior methicillin-resistant Staphylococcus aureus (MRSA) nares colonization was highly associated with odds 20 times higher of SSI following spinal fusion (OR, 20.30; 95% CI, 4.64–8.78).
CONCLUSIONSIn additional to nonmodifiable risk factors, prior colonization with MRSA is a modifiable risk factor very strongly associated with development of SSI following spinal fusion.

Density modulated nanoporous tungsten thin films and their nanomechanical properties

Density modulated tungsten (W) thin films with nanoscale porosity contents of 7% to 40% by volume were grown on Si substrates through magnetron sputter deposition. Process parameters were selected according to the structure zone model, which resulted in film thicknesses between 105 nm and 520 nm. Nanomechanical properties of samples were investigated by means of instrumented nanoindentation. Reduced-χ2 analysis was carried out to assess four models formulated through differential effective medium approach. The model that factored in both the crowding effect and the maximum random packing of pores successfully captured the experimental trends. Attempts to breach the auxetic barrier resulted in large-scale pulverization or spontaneous conversion into WO3. Porosity corrected yield strength calculations underlined the possibility of defining a porosity threshold beyond which the compressive yield strength of density modulated nanoporous metallic thin films would drop abruptly due to aggravated geometric slenderness effects in agreement with earlier hypotheses.

This note presents the first stratigraphic, paleoenvironmental, and systematic account of Silurian foraminifers in Wisconsin, which include nine previously described species of Ammodiscus, Glomospirella, Hemisphaerammina, Hyperammina, Lagenammina, Psammosphaera, Stomasphaera, Thurammina, and Webbinelloidea (Appendix 1). Early Silurian carbonates in this area (Fig. 1) were deposited in shelf and ramp environments in the western part of the Michigan Basin (Harris et al., 1998), and the foraminifers are associated with macrofaunal communities that represent Benthic Assemblages BA3 to BA5 of Brett et al. (1993).
Silicified agglutinated foraminifers were obtained from acidinsoluble residues processed for conodonts, sponge spicules, and macrofauna (Watkins et al., 1994; Kuglitsch, 1996; Watkins and Kuglitsch, 1997; Watkins and Coorough 1997a, 1997b). Silicification of the specimens, rather than a composition of agglutinated siliceous grains, is indicated by lack of detrital silica in enclosing dolostones, variable detail of preservation, and association with other silicified fossils.
Comparing the Accuracy of Three Pediatric Disaster Triage Strategies: A Simulation-Based Investigation

Background
It is unclear which pediatric disaster triage (PDT) strategy yields the best accuracy or best patient outcomes.
MethodsWe conducted a cross-sectional analysis on a sample of emergency medical services providers from a prospective cohort study comparing the accuracy and triage outcomes for 2 PDT strategies (Smart and JumpSTART) and clinical decision-making (CDM) with no algorithm. Participants were divided into cohorts by triage strategy. We presented 10-victim, multi-modal disaster simulations. A Delphi method determined patients’ expected triage levels. We compared triage accuracy overall and for each triage level (RED/Immediate, YELLOW/Delayed, GREEN/Ambulatory, BLACK/Deceased).
ResultsThere were 273 participants (71 JumpSTART, 122 Smart, and 81 CDM). There was no significant difference between Smart triage and CDM. When JumpSTART triage was used, there was greater accuracy than with either Smart (P<0.001; OR [odds ratio]: 2.03; interquartile range [IQR]: 1.30, 3.17) or CDM (P=0.02; OR: 1.76; IQR: 1.10, 2.82). JumpSTART outperformed Smart for RED patients (P=0.05; OR: 1.48; IQR: 1.01,2.17), and outperformed both Smart (P<0.001; OR: 3.22; IQR: 1.78,5.88) and CDM (P<0.001; OR: 2.86; IQR: 1.53,5.26) for YELLOW patients. Furthermore, JumpSTART outperformed CDM for BLACK patients (P=0.01; OR: 5.55; IQR: 1.47, 20.0).
ConclusionOur simulation-based comparison suggested that JumpSTART triage outperforms both Smart and CDM. JumpSTART outperformed Smart for RED patients and CDM for BLACK patients. For YELLOW patients, JumpSTART yielded more accurate triage results than did Smart triage or CDM. (Disaster Med Public Health Preparedness. 2016;10:253–260)

Two words sum up the present situation of the Church, ‘ambivalence’ and ‘confusion’. One the one hand, we have hierarchical structures and inevitably, we are steeped in hierarchical language and assumptions. Beside this we have an increasing vocabulary acknowledging and inviting ‘participation’ and ‘equality’, issuing from the senior hierarchy and directed especially to the ‘laity’.
Take the New Code of Canon Law, for instance. Can 208 reads: Flowing from their rebirth in Christ, there is a genuine equality of dignity and action among all of Christ’s faithful. Because of this equality they all contribute, each according to his or her own condition and office, to the building up of the Body of Christ.
We might be moved, as we read, to examine whether we had been taking our baptismal responsibilities as seriously as we ought. However, should we glance back to Can. 207:
By divine institution among Christ’s faithful there are in the Church sacred ministers, who in law are also called clerics; the others are called lay people, (my italics)
our sense of urgency might dwindle somewhat ... especially when we consider that ‘the others’ form something like 98% of the total. If what we, the ‘laity’, do is not by divine institution and our ministry not sacred, but rather is other, then our sense of participation, our sense of responsibility is diminished. We are diminished. Similar examples abound in all Church documents.
